We look forward to welcoming you and your family to Larson Eye Care. Your comprehensive eye examination and required testing will take approximately one hour, depending on your appointment type. Post-operative care examinations usually take an average of thirty minutes.

Your exam may require that your eyes be dilated. Please bring the following information with you to every appointment:

1. A complete list of current medications, including your dosage and how often you take each medication.
2. All current eyewear including your eyeglasses, contact lenses and your latest eye wear prescription.
3. All insurance plan cards including Medicare and private pay coverage.
4. A referral form from your primary care doctor if required by your insurance carrier.

All payments, including insurance co-payments and deductibles are due on the day of your visit. For your convenience we accept cash, personal checks, VISA, MasterCard, and Discover.
